The FLS Baix Vallès covers 168,000 inhabitants of 11 municipalities of the Vallès Oriental i Occidental region (Baix Vallès).
It is made up of three healthcare areas: -Hospitalisation Area (Mollet acute hospital with orthogeriatrics, traumatology, medical day hospital and monographic consultation of the Osteoporotic Fractures Unit (Rheumatology/Geriatrics). -Social and health area (Mollet Social and Health Foundation, with Medium Stay and Rehabilitation Unit devices). During admission, the patient will be assessed within the fall prevention program. Group of experts on the prevention of falls in the hospital area offer information on the discharge of fall prevention, written in the report or with informative material.
Primary Care Area in which they are integrated into the eight primary care centers from the Baix Vallès region.
Hospital Coordinator is part of the Rheumatology Department, but the FLS Care Committee is formed by 2 Rheumatologists, 2 Geriatricians, 1 Traumatologist , 1 Rehabilitator, 1 member of Family and Community Medicine), 2 Nurses (one of them the Specialized Case Manager and another of Primary Care), 1 Pharmacist (one of them from Specialized and the other from Primary Care, 1 occupational therapist. This Committee meets periodically for the protocolization of the area of knowledge (osteoporosis, fractures and falls) and that of specialized coordination, social health and community circuits.
