Flinders Medical Centre

In 2014 the Rehabilitation, Aged and Palliative Care at Flinders Medical Centre established first in South Australia Fragility Fracture Service which provides a highly effective solution in bridging clinical and service gaps in management of fragility fractures. It is multidisciplinary service, which includes Rehabilitation Physician, Dietician, Nurse Practitioner and Registered Nurse, and functions as a " one stop shop". The aim of the service is to systematically identify, investigate, inform, initiate treatment and integrate care for all patients over the age of 50 who have suffered a fragility fracture withe the aim of reducing their risk of subsequent fractures. The service KPIs place significant emphasis on consumer centered care with 5I's' key objectives approach.
