We identify the patients with osteoporotic fractures. We treat their medical background, check a vitamin D level at admission and treat by  loading dose of vitamin D and confirn our target level of vitamin D.

Then we recommend to patient start a osteoporosis preventing medication, as we decide. In addition, we guide our patients to prevent falls and promote healthy lifestyle.

Near a date of discherhe, we contact with local ambulatory clinic and inform tham about our patient and our recommenfdation for prevent future osteoporotic fractures and maintain a medical follow-up for proceed a continuation of our preventive antiosteoporotic treatment, which include medicine, lifestyle, safe home adjustment, family support, nutrition and individual sport activities.
