Po Jen General Hospital

The Fracture Liaison Services (FLS) at Po Jen General Hospital in Taiwan is a community based case management model.

1. Identification:  Due to a small community hospital setting, we could easily found new cases from the admission patient list and the
outpatient department. We enrolled the patients with fresh hip fractures and vertebral fractures.

2. Registration: Our case manager contacted the patient and ask his or her will to participate in our FLS program. The
patient would sign an informed consent for FLS.

3. Treatment Initiationt: The patients were referred to the physicians with a specialty of osteoporosis to provide adequate
pharmaceutical and non-pharmaceutical treatment to the patients.

4. Maintenance and adherence:  Our case manager would remind the patients about the regular treatment and follow-up date.
