Concord Repatriation General Hospital
The Concord Hospital Secondary Fracture Prevention (SFP) program was initiated by lead clinician Professor Markus Seibel in 2005. This tertiary care centre is part of the Sydney Local Health District, Sydney, Australia and serves a population of more than 580,000. The Concord SFP program is a model A service and identifies, assesses and manages patients following a minimal trauma fracture. The program has demonstrated a relative reduction in re-fractures by 60-80% (Lih et al. 2011, Ganda et al. 2013) and demonstrated overall cost-effectiveness (Cooper et al. 2012). The SFP program captures patients with all types of symptomatic fractures. The lead clinician supervises a 0.5 FTE coordinator, a position fully funded by the hospital.
