Since 2015, all patients' medical records are fully digitalized. This large historic and continuously updated database enables a better understanding of the implementation outcomes of clinical guidelines.

Our FLS team has been using and continuously upgrading the hospital's databases for an improved and more efficient identification process of patients' risks of falls and fractures. The Israeli Digital Medical Information Network enables post hospitalization follow-up for almost all our patients.

These disruptive innovations are the foundation for an excellent continuity of care within all CHCS' points of care, as well as with other health care providers in Israel.
