We are a hospital-based FLS but we cover a wide geographic area, including 2 hospitals, with an often rural and elderly population.  Our service includes fragility fractures of hip, wrist, proximal humerus, lumbosacral vertebra, and pelvis for people 50 yrs old and over. Our case finding methods cover both inpatient and outpatient / community. We are well connected with hospital and community services and networks, and work in partnership with other community-based health practitioners (physicians, allied health professionals, falls prevention groups, etc). We are part of a national network of FLSs (Fracture Liaison Network New Zealand) and are supported by Osteoporosis New Zealand and our national insurer (ACC). We utilize a registry as both a patient management system but also for data gathering and analysis.
