The Fundación Jiménez Díaz University Hospital is the reference hospital for 451.000 inhabitants of Madrid. The Multidisciplinary Fragility Fracture Prevention Unit provides comprehensive care for patients with fragility fractures in three circuits:
- The Multidisciplinary Spine Pathology Unit treats patients with vertebral fracture.
- Patients with hip fractures are admitted to the Department of Orthopedic Surgery and Traumatology and are evaluated and treated during admission by the Orthogeriatrics Unit (Geriatrics) and Internal Medicine through Shared Care.
- The remaining osteoporotic fractures are identified in the Orthopedic Surgery and Traumatology and Emergency Departments. Subsequent evaluation of fracture risk and treatment of osteoporosis is carried out by the Department of Rheumatology and Bone Metabolism Unit of Internal Medicine.
Within the continuity of care, we work specifically on the continuity of care of all the processes that require it with direct communication between the continuity of care nurses of the hospital and the nurses of the primary care centers.
