Taranaki, nestled into the west coast of the New Zealand has just celebrated one full year of being live on the Fragility Fracture Registry. Te Whatu Ora Taranaki serving a population of around 118,000 across the Taranaki region. Taranaki fracture liaison service deliver care for fragility fracture patients and service development of a secondary fracture prevention. This work is extremely rewarding as it contributes to a better outcomes and quality of life for patients. As a team we have had to work through how best to identify patients and as a team provide the best level of care for our patients. Our service contacts people over the age of 50 years who have a fractured bone following a low impact accident or fall. Our service offers future fracture risk assessment, falls assessment and information, health education about maintaining bone health, referrals to falls prevention coordinator, refferrals to community strength and balance classes, referring to bone density scans and identification of Osteoporosis. Our collaboration between New Zealand’s Accident Compensation Corporation, Te Whatu ora Health New Zealand, general practitioner, Taranaki Community Strength and Balance team and Osteoporosis New Zealand helps to deliver the level of care we provide.
