Fracture liaison service ensure fracture risk assessment, and treatment where appropriate, is delivered to all patients with fragility fractures. A fracture liaison service follows patients sustaining fragility fractures and/or osteoporotic fractures from the time of injury presentation until care is transitioned to the primary care provider. It is an interdisciplinary service combining orthopedic surgery, primary care, osteoporosis experts and ancillary services like physical therapy and dieticians to ensure that patients are properly assessed and managed. Criteria for enrolment into an FLS included being older than 50 years old and presenting with a fragility fracture.
IT system provided a pop up on discharge for patient admitted with fragility fracture, this pop up remind the orthopedic surgeon to refer the patient to osteoporosis clinic in primary healthcare. once referral done, the Fracture liaison coordinator will be notified and will contact the patient, explain the service and book appointment for osteoporosis clinic.
